Large Big Bariatric

How does one qualify for bariatric surgery in the Omaha Nebraska area?

I am 6'6 and 420 lbs recently I have pack on alot of weight due to quiting smoking and working at a pizza parlor with poor eating habits. I have always been large and I am ok with being big. However 420 pounds is life threatening. I have 2 children and 1 on the way and I want to be healthy for them. I have tried to shed the weight but everything I lose is back in a couple months. I have poor credit but Blue Cross Blue Shield insurance. Any suggestions?
